St. Jude Catholic Church, our charter organization, requires that any parent participating in a scouting event complete the required diocesan training. Please use the links below to complete the three requirements:

1. EAPPS Volunteer Application form

a) Complete address and phone number required for both references.

b) On the final page, each declaration must be initialed by hand and the signature needs to be handwritten.

2. Code of Ethics form

3. Complete course titled "Safe & Sacred Training for Adults" located under Certifications

a) You will need to create an account here.

b) Under training, take the course labeled “Serving Vulnerable Populations and the Diocesan Code of Ethics” (image)

c) After watching the presentation and completing the quiz, please save the certificate. (image)

NOTE: If you have already completed this training through St. Jude School, simply complete an authorization form allowing the church office to access your school file and link it to the church, and sign a new Code of Ethics. If you have completed this through the church already, let us know and we will confirm it with the church.

Once you have completed all three items above, please email them to Michelle Thompson at mzerin3@bellsouth.net and copy Beau Champion at bchamp1@gmail.com. Once received, they will be forwarded to the church office and you will be marked complete on our master list.
